I have to activate layer state twice to fully change between layer states. Doing the restore just once, leaves some layers turned on from the previous state. I have tried deleting and re-saving the states but it does not fix the problem.
What do you mean by “active layer state” and “restore layer state”? Do you mean change which layer is the current layer? Which layers are turned off?
Also what version of Rhino are you using and what operating system?

Would be great if there was a list somewhere of who to tag for certain suggestions / bugs.
Unfortunately I cant share the file, and I had realized recently that posting system also might be a security risk as it reveals information about O/S, hardware and computer username etc.
If I had time, I would try and strip the file, but that would take a while. If the problem happens again in any new files that I am working on I’ll comment back on this thread.
If anyone else has that issue I am sure that they will too.
There are some other threads on here regarding drafting/layout improvements and the need for an overhaul. Rhino is being used by practically every major architectural firm worldwide, I would be curious to know, how much of your customer base are Architects and Architecture firms. How much is/will Rhino development will be orientated towards architecture and engineering?
