Writing data from Grasshopper to Excel

I have a question regarding writing data from Grasshopper to Excel. I understand how it is done i.e. using stream destination and stream contents, however I would like to specifically create two columns representing information pertaining to the paths identified in a pane (please see the panel below and an attached Excel file showing the Grasshopper and the desired output). As shown in the Excel sheet, I would like one column to represent the path number and the other to show the data. Please let me know if you have any questions.
