From your image, I think you mean menus, not toolbars… perhaps correct the title and your message…

If you run the Menus command in Rhino do you see the duplicate entries in there? You can probably delete them if so, but I don’t know how they got there in the first place.

Only show once, what I have noticed is that I can only select the bottom item in the drop down list, if I select either of the first two the window closes.

We’re finding the same thing with the Orca3D menu, and when we run the Menus command and select Orca3D from the dropdown the menu editor closes.

Same here. Is there any fix for this?

Rhino 8 also occasionally crashes when I just
hover the mouse over one of those menus.

This should be fixed in the latest 8.3 release candidate (Jan 2 build)
