Fields showing wrong values: I must be doing something wrong?

Within the dialog it displays the correct values, but when the text lands in my model the values are way off. Am I losing my mind here? Any ideas?

I see that, thanks for the report.
@keithscadservices - as far as I can see, the problem lies in the annotation style.
