Does Rhino see a template file as a distinct file type, or only know that a file is a Template because it’s stored in the folder nominated in Rhino Options > Files?

Just to elaborate a bit on that one. There is, as Japhy wrote, no difference between template files and any other 3dm files. When you run New
, you are asked to find a 3dm file that will be used as a template (or you can use the “No Template” option). This file doesn’t have to be located in that path that is set in the Rhino options. You can use any 3dm file in any location.
The behavior has always been the same.
