Anyone have any experience using Rhino5 at 4k UHD 3840x2160 resolution? Does the UI scale properly?

AFAIK Rhino UI doesn’t scale. Icons are fixed in pixel size. They will not scale with the Windows UI scale. Text UI does scale and looks pretty right considereing the size mismatch between text-only Items and icon items.
Looks different, but great and still quite usable. You may need to tune up your mouse acceleration and it probably will take some getting used to in order to get your speed back hitting the toolbar icons.
