MeshTrim - why does it create so many extra faces?

In the file below, MeshTrim creates a whole bunch of fine triangular faces that weren’t there to begin with. MergeAllCoplanarFaces fixes it but why is it happening in the first place?

In addition, inside the red circle, there are 3 mesh faces that should be trimmed away but aren’t. OK, I know mesh trimming operations are not all that reliable, but this seems simple enough.
